This form needs to be filled out on your PC and printed, then signed and dated. · Confidentiality and Non-Disclosure Agreement (Page 1 of link). Print, sign and date. · NCCI Contractor Identification Card: Policy and Conduct Agreement (Page 2 of link). Print, sign and date. · Copy of ID (driver’s license). · Testing 1. FDCPA (the reading material is found on pages 3-22 of link) § FDCPA Exam #1 For Reps § FDCPA Exam #2 For Sub-reps 2. Physical Onsite Inspection (reading material and exam) · Copy of business documentation o Sole Proprietorship/DBA, LLC, S Corp or partnership · Copy of void business check · W-9 form with EIN # (Not Social Security #).
